Since ShockBox's server creation documentation is kinda scrappy, I threw this together to assist. The creation of a server is very simple, and I made this as total GPL licensed freeware, not a comprehensive creation package, so don't expect perfect documentation.
First off, create a folder, which will be your root directory. Then, create one folder in that folder for each album or collection you wish to offer. Then, place your encoded SWA, MP3, or whatever files inside those folders.
For advertisements, create a folder called "advert" and place ads in it. See the ShockBox documentation for size information.
Now then.... Open ServerMaker, click "Select Folder", and attempt to save the file inside your root folder. The file won't be created; It's simply used to mark where the root folder is. Then, ServerMaker will attempt to load all the files up. If there are no folders to scan, it might crash, so sue me.
Click in the right field to change the info of the various files. If the files are in the advertisement folder, the info will be used as the URL. Convenient, eh?
Finally, when you are done creating the info, click "Generate Server Files..." and it will process the items and output the server files.
Sometimes, ServerMaker will pick up CD cover images and such, or previously generated server files. Sometimes, it won't put things in the order you want. That's what Remove and the up/down arrows are for. :-)
Advanced Instructions
If you've already created a project file (you can save your server projects by going to File -> Save Server…), you can reload it and edit it without losing all of your previous data.
To add an album, click "Add Album" and select an album from the list. It will add an album entry so you may add more songs.
To add a song to an existing album, select it and click "Add Song" Then, select a song, and click "Add" to add it.
Fun feature implimented in 1.1: "Import CD cover" uses QuickTime to load an image you select, uses QuickDraw to scale and anti-alias it so it looks really nice, and then uses the QuickTime JPEG library to re-save it. Of course, if the scaling results don't look that great, you may click "No" when asked if it looks acceptable. To import a CD cover, select an album, click "Import CD cover", select an image, and click "Yes" when asked if it looks good.

Source code is included, so if you want one, make one.

Now then... Since that's all explained, read COPYING for info on distributing this and the source code. For the most part, if this package is distributed with source code unmodified, fine. Don't charge people for it, unless it's packed with all other sorts of goodies and actually cost you something to create that pack (i.e. shareware CD-ROM) or I'll break your neck.
